Highmark PPO Blue
- Highmark PPO Blue coverage is effective April 1, 2022
- Pharmacy benefits are included with the Highmark PPO Blue plan
- Payroll contributions are unchanged from 2021 & based on annual budgeted income
- Payroll contributions are the same for Gannon Select Plan and Gannon Standard Plan
- Includes vision coverage – Fashion Focus Gold Plan; Davis Vision network
- Provides eye exam and frames and lens or contacts every two years
Highmark PPO Blue Gannon Select Plan
- HDHP (High Deductible Health Plan)
- Health Savings Account (HSA) provided by Gannon*
- Gannon contribution of $1,150 annually individual; $2,300 family
- 60% funded in April; 40% funded in July
- Reduces out of pocket deductible to $350 (from $1,500) -
individual coverage & to $700 (from $3,000) - family coverage
- Employee contributions allowed, investment option, money does not
need used up at the end of the year
Highmark PPO Blue Gannon Standard Plan
- Lower deductibles - $350 individual/$700 family*
- Flexible Spending Account (FSA) optional (no Gannon contribution)
- Must be used by the end of the benefit year; no changes allowed
Spousal Coverage Surcharge: Effective with each new plan year (April 1 through March 30), when a colleague's spouse is eligible for coverage under his/her own employer's health insurance plan but elects to enroll or maintain enrollment in Gannon's health insurance plan, an additional $50 per pay (or $100 per month) will be added to the "Colleague Plus Spouse" or "Family" premium rates.
